2) Evaluate the validity of multiple algorithms for identifying disease cases from Manitoba administrative data.

IOM - Preventing Medication Errors
This report from the Institute of Medicine finds that medication errors are surprisingly common and costly, and it outlines a comprehensive approach to decreasing the prevalence of these errors.

AHRQ - DoD Medical Team Training Programs
Three medical team training programs based on aviation team training are evaluated under a contract from the Agency for Healthcare Research and Quality (AHRQ).
Results suggest that all three programs have desirable traits such as using active learning techniques and offering training to interdisciplinary health care teams. The researchers set out a framework for a successful medical team training program.

The Committee found that in 2004-05 some 974,000 patient safety incidents and near misses were recorded on NHS trusts' reporting systems. NHS trusts need to bring down the level of avoidable incidents, particularly those leading to serious harm and death, through rigorous implementation of safety alerts and adoption of high impact, evidence based solutions such as those promulgated by the National Patient Safety Agency and the Institute of Innovation and Improvement.

MCHP - Application of Patient Safety Indicators in Manitoba: A First Look
Given the potential contribution of administrative data to patient safetyrelated policy and practice, the Manitoba Centre for Health Policy (MCHP) set out to develop patient safety indicators using the MCHP Population Health Research Data Repository (Repository).
Indicators were selected and developed based on a review of the literature, the feasibility of using administrative data, and the input of physician Working Group members.
Based on these criteria the following indicators of compromised patient safety were selected for the report: (1) A selection of “Patient Safety Indicators” (PSIs) developed by AHRQ in the U.S., and (2) measures of complications related to laparoscopic cholecystectomy (removal of gallbladder).

The Care of Patients with Severe Chronic Illness: An Online Report on the Medicare Program
A new study by the Dartmouth Atlas Project at the Center for the Evaluative Clinical Sciences (CECS) at Dartmouth Medical School report that:
- The care of people with chronic illness accounts for more than 75% of all u.s. health care expenditures, but Medicare spends much more per enrollee in some states and regions than it does elsewhere.

CMWF - Committed to Safety: Ten Case Studies on Reducing Harm to Patients
This report from The Commonwealth Fund presents 10 case studies of health care organizations, clinical teams, and learning collaborations that have designed innovations in five areas that hold great promise for improving patient safety:
- promoting an organizational culture of safety,
- improving teamwork and communication,
- enhancing rapid response to prevent heart attacks and other crises in the hospital,
- preventing health care–associated infections in the intensive care unit, and
preventing adverse drug events throughout the hospital.
