CMS announces payment reforms for inpatient hospital services in 2008




Press Release

The Centers for Medicare & Medicaid Services (CMS) will reform the acute care hospital inpatient prospective payment system (IPPS).

The IPPS payment reforms would restructure the inpatient diagnosis-related groups (DRGs) to account more fully for the severity of each patient’s condition.

In addition, the rule includes important provisions to ensure that Medicare no longer pays for the additional costs of certain preventable conditions (including certain infections) acquired in the hospital.
To implement this policy, hospitals will be required to report secondary diagnoses present during admission, making it possible to detect what conditions were acquired during hospital stays.

The rule also expands the list of publicly reported quality measures.

The New York City Health and Hospitals Corporation (HHC) released data to the public on safety and quality indicators:

  • Annual mortality rates for the entire system and for each of its 11 hospitals;
  • Adherence to best practice treatment for patients with heart attack, heart failure and pneumonia;
  • 30-day mortality rates for heart attack and heart failure;
  • Adherence to best practice treatment for prevention of surgical infection;
  • Rates of ventilator-associated pneumonia and central line-associated bloodstream infection for critically ill patients;
  • Rates of falls, pressure ulcers and effective pain management for long term care patients.

Speakers from this Web conference discussed the experiences of their HIE organizations in implementing the the Ambulatory Care Quality Alliance (AQA) "starter set" of 26 clinical performance measures for the ambulatory care setting.

The starter set of measures is intended to provide clinicians, consumers, and purchasers with a set of quality indicators that may be used for quality improvement, public reporting, and pay-for-performance programs.
