The lack of a framework on how to define and classify patient safety events, healthcare errors, and systems failures is a major barrier to the systematic understanding of where and how such problems occur.

This National Quality Forum (NQF) report presents a set of four voluntary consensus standards around a specific patient safety taxonomy—the Patient Safety Event Taxonomy (PSET).
